Report elaboration for the client was based on evaluation of

• Physical inspection of the plant after accident
• Assessment of existing technical reports related to the inspections performed by other third parties
• Analysis and evaluation of process parameters recorded by DCS at the time of accident and/or records from operation (log books) with the aim to identify root cause of the accident and possible course of process before, during and after event
• Assessment and evaluation of former contractor´s documentation related to the project execution since its beginning up to now
• Preparation of technical report with results of assessment
• Support to the Client in preparation of arbitrary case proceedings with former EPC contractor Site supervision and Technical assistance during rehab works at the plant
• Design review
• Erection and commissioning supervision and advisory for the Client
